On a June 23, 2003, a Notice of Violation and Proposed Imposition of Civil Penalty in the amount of $60,000 was issued for the willful failure to provide complete and accurate information to the NRC Region III staff concerning the high pressure coolant injection (HPCI) system during a telephone conference call on September 27, 2001. A Notice of Violation was also issued for a violation associated with a White SDP finding.
